Recovering From Total Knee Replacement: A Week-by-Week Timeline

What to expect after a total knee replacement, from the first 24 hours to full recovery at 6–12 months — a realistic week-by-week guide.

Knowing what to expect after a knee replacement removes a lot of the anxiety. Every patient is different, but most follow a broadly similar path. Here is a realistic week-by-week guide to recovery after a total knee replacement.

The first 24 hours

You will usually stand and take a few supported steps with a walker on the same day or the day after surgery, with physiotherapy support. Pain is controlled with a planned combination of medications. Early movement is encouraged because it reduces stiffness and the risk of clots.

Days 2–3: going home

Most patients are discharged once they can move safely with a walker, manage a few stairs, and have well-controlled pain. You will leave with a clear physiotherapy plan and instructions on wound care and medication.

Weeks 2–6: building momentum

This is the most important phase. Swelling gradually settles, your walking distance increases, and your knee bends a little further each week with guided exercises. Sutures or clips are typically reviewed around two weeks. Doing your physiotherapy consistently in this window has the biggest influence on your final result.

  • Use ice and elevation to manage swelling.
  • Walk little and often rather than overdoing it in one go.
  • Keep up the prescribed bending and straightening exercises daily.

3 months: back to routine

By around three months most people have resumed everyday activities — driving, shopping, light work and social outings — and are continuing to build strength. Some swelling and occasional warmth around the knee is normal and continues to improve.

6–12 months: the full benefit

The knee continues to strengthen and feel more natural over the rest of the year. By 6–12 months you should be enjoying the full benefit of the replacement, after which an annual review keeps an eye on the implant.

When to call the clinic

Recovery is rarely a perfectly straight line, but some signs deserve prompt attention: increasing redness or discharge from the wound, fever, calf pain or swelling, or pain that suddenly worsens rather than improves. If in doubt, get in touch — it is always better to check.
